Anyone have a templete for a HealthCare Provider Database?

Hello, just looking for a templete that trackes medical providers and updates
etc. Instead of having to create from scratch. Thank you anyone out there
who has something that would work. We can change the database as needed, but
would like a template to start. Thank you!!

Have you checked on-line? Have you checked at Microsoft.com?

Which country are you building this in/for? If the USA, will you need to
ensure/document that your application is compliant with all regulations?

Thank you for your response. Yes, I searched the access templates on
microsoft, but not related to tracking providers/changes. This would be
compliant w/ all regulations as this would be an internal provider directory.
Its, not that major. Thank you! Basically need to track, names address,
all provider demographics, and changes and updates.

That sounds like a Contact database. Maybe look for templates with other
search terms...
